A significant clinical trial conducted by the All India Institute of Medical Sciences (AIIMS) has found that digoxin, a low-cost heart medication, may help reduce the risk of worsening heart failure in patients suffering from rheumatic heart disease (RHD). RHD affects over 40 million people globally, leading to nearly 320,000 deaths annually. The study, published in the Journal of the American Medical Association (JAMA) on May 10, involved 1,769 adults with symptomatic RHD from 12 tertiary care hospitals in India. Participants were randomly assigned to receive either digoxin or a placebo alongside standard treatment. Results showed that 25.8% of patients on digoxin experienced worsening heart failure compared to 29.2% in the placebo group. While the study did not find a significant difference in overall mortality, it reported a low incidence of suspected digoxin toxicity, with only 24 patients experiencing suspected side effects. The findings suggest that digoxin could be a safe and affordable option for managing heart failure in RHD patients, especially in low- and middle-income countries.
